Position Summary:
As an MDS Coordinator (RN), you will play a crucial role in ensuring accurate and comprehensive assessments of our residents' healthcare needs. You will work closely with our interdisciplinary team to develop individualized care plans, coordinate care services, and maintain compliance with regulatory requirements. Your expertise will contribute to enhancing the overall quality of care provided to our residents.
Responsibilities:
- Conduct thorough assessments of residents' healthcare needs and develop comprehensive care plans.
- Coordinate and communicate with the interdisciplinary team to ensure the timely implementation of care plans.
- Monitor and evaluate the effectiveness of care plans, making necessary adjustments as needed.
- Ensure accurate and timely completion of the Minimum Data Set (MDS) and other required documentation.
- Collaborate with healthcare professionals, including physicians, to ensure optimal resident care.
- Stay up to date with regulatory requirements and ensure compliance with state and federal guidelines.
- Provide education and support to staff members regarding MDS and care planning processes.
Qualifications:
- Valid Registered Nurse (RN) license in the state of Indiana.
- Minimum of 2 years of experience as an MDS Coordinator in a skilled nursing or long-term care setting.
- In-depth knowledge of the MDS assessment process and care planning.
- Strong understanding of Medicare, Medicaid, and other reimbursement systems.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively as part of an interdisciplinary team.
- Detail-oriented with strong organizational and time management abilities.
- Proficiency in using electronic medical record (EMR) systems.
