We’re excited to help our residents safely reunite with their families through our secure indoor visitation policy. For now, we are requiring a maximum of 2 guests per visit (aged 12+). Please see the steps below to coordinate your visit to our community.

SCHEDULING OF VISITS: All visits at Kingston Care Center of Fort Wayne will need to be scheduled. Please schedule online by clicking here to book your visit, or call Michelle at 260-489-2552, Monday-Friday, from 9am-4pm.

PUBLIC VISITING HOURS: 9a-5 p on Mon, Wed, Thurs, Fri, Sat and 11-7 pm on Tues.

FREQUENCY OF VISITS: Visits will begin slowly and increase with time. To begin, we are asking that you not schedule more than 1 visit every 1 weeks, so we can ensure all our residents our able to receive visitors.

ENTRANCE: At the door to our “West Sun room” exterior exit. Please ring doorbell at the entrance.

PARKING: 4 spaces have been reserved for 'visitor parking'-located near entrance of visiting area.

VISIT LENGTH: Length of visitation times may vary, please contact our community or refer to our online scheduling site for the latest information

NUMBER OF VISITORS: Limit of 2 assigned visitors per resident. Visitors must be at least 12 years of age and able to follow social distancing requirements and wear a surgical grade face mask. No pets allowed at this time.

VISITOR ARRIVAL: All Visitors must be scheduled. Please plan to arrive 10 minutes early to ensure you are timely. All visitations must be approved based on COVID-19 questionnaire, and visitor signed understanding of risks and visitation guidelines, and signed/waiver consent.

  • Please arrive with personal face mask/face mask must be kept on during entire visit
  • UNVACCINATED RESIDENTS: Please be responsible for following 6-ft social distancing No kissing, hugging, touching
  • VACCINATED RESIDENTS: May hug loved ones if both resident and visitor wear surgical grade face mask
  • Staff will check you in and ensure Visitor Guideline Consent, Waiver, and health screen are completed and guide you to visitation area and bring your loved one to you, or you to your loved one
  • Staff member will take your temperature at the front area before entering the building
  • Please use hand sanitizer supplied at the entrance, prior to seating.
  • In some cases, you may be asked to wear a washable gown to help protect the residents.
  • Staff will escort you to the “West Sun-Room” visiting area. Please stay in designated visitor area
  • Visit will be timed according to schedule
  • Once visit has concluded, please use hand sanitizer and staff will assist you with exiting visiting area
  • Plan to monitor self for signs/symptoms of COVID-19 for 14 days following visit

GIFT ITEMS: No food/beverage/tobacco/vaping shall be permitted during visitation. All items brought from home for a resident must follow Kingston’s policy for allowable items, including location of drop off, typical procedures for deliveries, sanitation.
