We’re excited to help our residents safely reunite with their families through our secure outdoor visitation policy - beginning September 1st, 2020. For now, we are requiring a maximum of 1 guest for each visit. Please see the steps below to coordinate your visit to our community.
Step 1: Visitation Decision Tree

Step 2: Health Assessment
Visitors (maximum of 1 per visit) should print, fill out and and bring our health assessment form, click here to access. If you are unable to print this form, we will provide you a blank form for you to review and sign upon your arrival, please plan to arrive in advance of your scheduled visit to complete. Temperature readings will be performed on site. Thank you for your honest responses so that we can continue to protect our residents and staff from risks of exposure to Covid-19.

Step 4: Coordinating Your Visit
Please call 505-471-2400 Monday thru Friday 9a-4p. You may also click here to schedule your visit by choosing a time slot and entering your name and contact information.
In compliance with New Mexico Department of Health’s mandate, we must require that you schedule no more than 1 visitation per month. Additionally, we can only allow 1 visitor per resident. We are hopeful to expand visitation allowances as soon as it is safe to do so.
So that we remain in compliance with HIPAA, we recommend that you do not include the name of the resident you are visiting in your reservation. Instead, please include the room # of your resident to help us coordinate their schedule. If you do not have this information, please reach out to our front desk.
Inclement Weather: Outside visitation may be rescheduled due to inclement weather. If your visit is scheduled during potential inclement weather, please call Kingston Scheduling to ensure visitation has not been cancelled.
Pet Policy: While we would love to invite your furry friends to join us, we are currently restricting pets from visits to our community. We will revisit this guideline as we continue to monitor and expand our visitation allowances.
